Parents APPLY to the school for admission, then if all is in order, their child is ACCEPTED to the school, and ultimately they REGISTER.  Applicants must meet the school’s academic, psychological and residential requirements in order to register. All decisions about student admission are made by the Principal.

SAINT DENIS-SAINT COLUMBA SCHOOL accepts applications for prospective students entering Grades K-8.  Application Forms and Tuition Information are available below.  Simply click on the links below to view and print each document. 

REQUIREMENTS FOR ADMISSION include the following documentation:

• Baptismal Certificate

• Birth Certificate

• Social Security Card/Number

• Immunization Records

• Health Information (Health Data Sheet and Physical Form)

• Recent Report Cards

• Recent New York State Tests in ELA and Math
  Recent Standardized Tests (ITBS, etc.)

• Parish Envelope Number (must be an active parishioner at St. Columba, St. Denis, or Blessed Kateri with a history of contributions prior to registration in order to be considered for the in-parish tuition rate.)

• Sacramental Information, including a copy of child's First Communion,
  Reconciliation, and Confirmation certificates

• Two Letters of Recommendation from Principal and Teacher (Grades 3-8)

• Transportation Forms (available in the school office upon registration)

• $100.00 (non-refundable) Application Fee

• $200.00  General/Technology Fee and PSA/Supervisory Fee (see Tuition Rates)
