They may hurt for a second, but an immunization is your child's best shot at good health!

New York State Law requires that your child have certain immunization on file in the Health Office to attend school.

NEW FOR 2016: Students entering Grade 7 must receive an adequate dose of the meningococcal vaccine (meningitis).  For more information, click HERE.

DTaP/DTP (Diptheria, Tetanus and Pertussis)

Grades 1-5

5 doses (4 if last dose was received after 4th birthday)

Grade 6

3 doses

IPV or OPV (Polio)

Grades 1-5

3 doses

4 doses (3 if last dose was received after 4th birthday)

Varicella (Chicken Pox)
